AffinityWorks' Commitment to Privacy Effective 1/1/2002 Hypnotherapy Services: please see note on HIPPA below Your Privacy Is Important! This notice is provided to protect your privacy. It explains the information practices used by AffinityWorks and helps you to make choices about the way your information is submitted and used. A link to this notice is available from our homepage, as well as other areas where you might give personally identifiable information to us. Information You Submit to AffinityWorks This privacy policy applies to all information you provide by any method, whether by phone, mail, in person, or within the AffinityWorks website. On some pages you can email or call to request services, products, or make personal inquiries. The types of personal information you might submit to us are name, address, email address, and phone number. The Way Your Information Is Used The information you provide about yourself when requesting services, products, or making inquiries is used only to complete your request. We do not share this information with any outside parties. This policy is strictly enforced. Return email addresses are used to answer inquiries we receive from you, or to deliver the services and products you request. Phone numbers and mailing addresses may be used to complete your payment information, and also to deliver goods and services to you. These steps are important to prevent fraud. Email addresses, phone numbers, and mailing addresses are not used for any other purpose, and are never shared with any third parties for advertising, promotional, or statistical purposes. Please note: When you connect to a website outside of AffinityWorks via any of its links, you are protected by the privacy policy of that website or business and not our policy. For example, this would apply when you link to Amazon.com to place an order. AffinityWorks' Commitment to Children's Privacy We feel that protecting children and their privacy is especially important. Therefore, we never accept the submission of information to our website, including requests for goods and services, from anyone actually known to be under the age of 18. Also, no part of AffinityWorks' website has been structured to attract anyone under 18 years of age. A Note on HIPAA Since April 2003, the U.S. government has required regulated healthcare professionals to conform with The Health Insurance Portability & Accountability Act (HIPAA). At this time neither "hypnotherapists" or "hypnotists" are considered to be healthcare professionals. For more information on HIPAA, please consult this fact sheet from the U.S. Department of Health & Human Services. When you receive hypnotherapy services from AffinityWorks, a Client Bill of Rights is provided that further addresses confidentiality issues. How to Contact AffinityWorks If you have any questions or concerns about these privacy policies, or regarding information you may have already provided us, please call AffinityWorks at 860-301-9064 or send us a confidential email.
